AIR CARRIER NOTICE
Air Critical Care® is an authorized Indirect Air Carrier (IAC) and does not employee any flight crew. Air Critical Care® arranges flights under an agreement with Express Air Medical that are Direct Air Carriers authorized by 14 CFR Part 135 and all medical services are that of Air Critical Cares staff. Air Critical Care® may utilize other air carriers under Part 135 and Part 121 to meet your Air Ambulance needs.
PERSONNEL
In some cases, the team members that may accompany a patient may not be an employee of Air Critical Care® LLC or ACCMedlink™, we may utilize leased or contracted team members for some patient care services. Any rights not expressly granted are herein reserved. We are not licensed to perform emergency care in our long distance medical transport vehicles. ACC Medlink™ medical transport is not an ambulance service.

PRIVACY POLICIES
Health Insurance Portability Accountability Act of 1996 (HIPAA) is governed by the U.S. Dept. of Health and Human Services, office of Civil Rights and regulates the act that protects the disclosure of consumers medical records. Air Critical Care LLC. will keep your information confidential and protected. We will follow our privacy practices to insure that your personal medical information is protected. The Privacy Policy for Air Critical Care LLC, outlines how your medical information will be protected and what your rights are to the use and disclosure of your medical information. Understand that under HIPAA all individuals and organizations that are involved in the patient’s healthcare are required to follow the HIPAA privacy practices. Information Collected: Note that all and any information collected from our website will not be used for any other purpose but, the medical transportation service offered on this website. We do not retain any email records of our customers for more than 30 days post service renderend date.
DISCLOSURE
We may disclose information to other healthcare organizations as required for the patients’ healthcare, medical billing and heath care operations for quality improvement practices. We may be providing information to your healthcare insurance carrier for claim processing. These healthcare providers are obligated to the same requirements to keep you records private and confidential. We may be required by law to disclose medical information in response to a request from the government, law enforcement and or legal judicial orders. We may disclose information to a friend or family member who is assisting with your medical care, or maybe providing financial arrangements on your behalf. We will not disclose or release your information to anyone else without your written permission or prior authorization.
REQUESTS
You have the right to request a copy of your records and/or request changes to your records if you feel there is inaccurate information we have on record.

Air Critical Care LLC. retains the rights to make changes to this policy. Patients will receive a copy at the time services are rendered however, will not receive the updated policies but, may request a written copy at anytime by contacting Air Critical Care LLC. directly at 800-550-0674.
COMPLAINTS
You have the right if you feel that Air Critical Care LLC. has violated the privacy of your medical records, to file a written complaint with the United States Department of Health and Human Services, or for guidance you can reference www.HHS.gov.
